Required BSA Medical Forms 2020

These copies of the medical forms are valid as of 6/1/2020. Since medical restrictions are evolving rapidly due to the pandemic you may wish to consult the BSA website for changes. All scouts and leaders must have completed Forms A and B registered with the troop/pack/crew. Scouts and leaders participating in week-long activities (e.g. summer camp) must also have a completed Form C registered with the pack.
